Sometimes the most important place to do physiotherapy is where you actually live. River Physio’s home rehabilitation service brings individualised care directly to your home — supporting you during the transition out of hospital, into aged care, or while managing complex conditions where getting out is difficult.
With over 10 years of experience working with people living with dementia, cognitive decline, Parkinson’s disease, and culturally and linguistically diverse communities, Tatiana takes a holistic approach to supporting both you and the people around you. The goal is always the same: helping you maintain independence, dignity, and confidence in your own environment.
What’s included
- Functional assessments — understanding your current capacity, fall risk, home mobility, and daily activities
- Walking aid prescription and fitting — canes, frames, wheeled walkers, and other mobility equipment
- Manual handling and carer training — safe transfer techniques, equipment use, and reducing carer injury risk
- Falls assessment and prevention — individualised exercise programs and home environment review
- Frailty and sarcopenia management — progressive strength and conditioning designed for older adults
- Community access and outdoor mobility training — practising real-world environments like shops, parks, and public transport
- Post-discharge rehabilitation — bridging the gap between hospital and home after joint replacement, neurological events, or complex medical recovery
Falls prevention
Falls risk can increase gradually over time. If you’ve noticed changes in stability, balance, or confidence on your feet — for yourself or someone you care for — a physiotherapy assessment can help identify practical steps to reduce risk and support safe mobility at home.
We offer falls assessments, individualised exercise programs, and home environment reviews as part of a proactive approach to staying safe and independent.
River Physio supports April Falls Month — an initiative of the NSW Fall Prevention & Healthy Ageing Network.
Supporting carers and families
A key part of home rehabilitation is equipping the people around you with practical skills. We provide:
- One-on-one manual handling training for family carers
- Aged care facility visits for staff training
- Equipment recommendations and demonstration
- Education on positioning, transfers, and falls prevention
